VO2 max is the single greatest clinical predictor of all-cause mortality reduction (hazard ratio ~0.20 for top vs bottom decile). Metformin inhibits complex I in the electron transport chain, directly blunting PGC-1alpha mitochondrial biogenesis and cardiorespiratory fitness gains in active adults. Sacrificing exercise-induced adaptations for a modest glucoregulatory benefit is counter-productive for fit individuals.

Human aging is driven by multimorbidity, not isolated muscle metrics. Metformin exerts broad systemic epigenomic, anti-inflammatory, and anti-neoplastic protections that go far beyond glycemic control. Even if it blunts peak exercise adaptation by ~5%, the systemic suppression of DNA damage and vascular inflamma-aging may extend healthy lifespan across populations.
